Delaware state law requires a physical examination, immunizations, TB Risk Assessment for all school pupils, as well as lead blood test documentation for children entering kindergarten. Please see the Student Handbook Health section for more details about these requirements.
Student Health History Update, this form is required for all new and returning CCSstudents.
Permission to administer over the counter medication by the school nurse. This form is required for all new and returning CCSstudents, even if you do not want your child to receive medication while at school. Also, no medications will be provided to your child unless this form is on file.
Emergency Health Care Plan. If your child has any medical condition that might require emergency care, a plan should be discussed with his/her doctor, and be on file in the nurse's office. This would include, but is not limited to, seizures, diabetes, allergic reactions that require an EpiPen, and cardiovascular problems.
Asthma Action Plan. If your child has a diagnosis of asthma, an asthma action plan (from your physician) must be on file in the nurse's office. Also, an inhaler/nebulizer should be kept in the nurse's office as prescribed. If your child is required to carry an inhaler on him/her while in school, please see me to complete the appropriate documentation.
Parental Request to Have Prescription Medications Administered in School. If it is prescribed for your child to receive medication during the school day please bring the medication to school in the original properly labeled container and complete the appropriate paperwork.
